These are the forms you will need to participate as a vendor in the California Utah Women's Fall and Spring Boutiques.
Application for the "Fall 2010 Boutique"
Table fee: $35.00 per table (tables are approximately 6'L 3'W)
This application must be filled in completely if you are planning to be a vendor at the boutique - even if you have participated in prior events. Your application must be received by July 15, 2010. Some applications may be accepted for consideration after the due date only if there is space available.
